Horizon Health Services: Stronger Together Community Networking Event

We are excited to host a community event to improve collaboration and engagement efforts for individuals across organizations throughout Western New York. The goal of this event is to build and strengthen our community network and in turn enhance the support and service availability for patients and community members.

We will discuss stronger efforts towards integrated care and focus on barriers to understanding health inequities in our communities.
